Rent Subsidy Application - Completed and signed by main tenant, includes household members details. 

NOTE: If household members have left evidence needs to be provided that they no longer live at this property, see Evidence Requirements section within the subsidy for more information.

Rent Subsidy - Centrelink Consent Form - To be completed by all household members 18+ that receive Centrelink benefits.

Payslips will need to be provided for all household members 18+ that work.
